Overview

The family outreach specialist assists families in connecting with community-based healthcare, human services, and community resources and provides in-home visitation sessions emphasizing child development, behavior management actvities, and parenting skills development

Responsibilities

Direct Patient Care

* Facilitate all referrals between families and community referral resources.
* Use a family-centered approach in providing support and assistance to families.
* Advocate for, educate and support families as they select and use effective resources for their needs.
* Maintain contact with clients by phone, correspondence, and/or home visits to assess needs to successfully link families to effective services.
* Assist families with scheduling appointments, finding language interpretation services, childcare, transportation and other resources as necessary.
* Provide support and problem solving to families, providers, community resources, and Trinity Muscatine Public Health (TMPH).
* Communication and Resource Responsibilities

Community and Program Involvement

* Attend project-related meetings as needed.
* Participate in community events providing program education as requested.
* Engage in on-going professional trainings.
* Maintains core competency training requirements.
* Perform other duties as assigned by manager

Qualifications

Minimum Requirements

Education:

Bachelor's degree in health education, counseling, nursing, sociology, or psychology; or a graduate of a Bachelor's degree program or higher in social work

Experience:

Demonstrated organization, communication and documentation skill required.

License(s)/Certification(s):

Valid driver's license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:

Knowledge of child health development

Knowledge of Community resources and services

Analytical Skills to interpret information

Ability to meet timelines/deadlines for reports and grants

Other:
